Purpose of this Form

This form is used to file inquiries, requests for assistance, suggestions, or complaints regarding frontline services, employees, or programs of the Local Government Unit (LGU) of Loon.

In compliance with the Republic Act No. 9485, also known as the Anti-Red Tape Act of 2007, the LGU-Loon is committed to promoting transparency and ensuring the efficient and prompt delivery of public services. Your feedback is vital in helping us identify areas for improvement and uphold our service standards as outlined in our Citizen’s Charter.

Who Can Use This Form?

Any individual, resident of Loon or not, who has transacted business with LGU-Loon or wishes to raise a concern can use this form.

How to Fill Out the Form

  • Provide accurate and complete details: This will help us investigate and act on your concern promptly.
  • Be specific: Clearly state the nature of your request, the specific office or personnel involved (if applicable), and the date and time of the incident or service you availed.
  • Attach supporting documents (if any): Include relevant evidence such as photos, receipts, or correspondence to support your complaint. (This is not included in the feature of this form but may be asked during our physical evaluation, if applicable)
  • Indicate your preferred mode of feedback: Let us know how you would like to receive our response (e.g., email, phone call, letter).

What Happens After You Submit the Form?

  1. Receipt and Logging: Your concern will be received by the PACD Action Officer and logged into our system with a reference number.
  2. Evaluation and Referral: The Action Officer will evaluate the concern and refer it to the appropriate department or official within LGU-Loon for action.
  3. Action and Feedback: The concerned office will address the issue. You will receive an official response or update within a maximum of five (5) working days upon receipt of the complaint, as mandated by law.

The personal information collected on this form (Full Name, Address, Contact Number, Email Address, ID Information) will be used solely for the purpose of processing your request or complaint.

All collected data will be handled with confidentiality and securely stored. You have the option to file a complaint anonymously, but providing contact details allows us to give you specific feedback and resolution regarding your concern.
